A 22-year-old primigravida woman presented in her 9th month (third trimester) of pregnancy with massive edema of the labia minora and labia majora [Figure 1], without any pain or tenderness, for 5 days. The patient had severe anemia, which had resulted in congestive cardiac failure. We made a diagnosis of vulval edema secondary to congestive cardiac failure. Vulval edema occurs in patients with severe anemia complicated by cardiac failure. Hypoalbuminemia can result in decreased oncotic pressure and increased capillary permeability.[1] Other causes of vulval edema in pregnancy include hypertension, hepatic abnormality, renal abnormality, sepsis, trauma, multiple pregnancy, and obstructed labor. It may progress if physicians do not identify the underlying cause and treat them.
